Families should be ready to provide:
- Player full legal name
- Date of birth
- Parent or legal guardian name and contact information
- Emergency contact
- Relevant medical or allergy information
- Signed tournament participation agreement
- Participation and assumption-of-risk acknowledgement
- Liability release
- Emergency medical authorization
- Photo and event-media choice
- Electronic signature and date

The final digital waiver will be sent after the coach submits the accepted team roster.
